Practice & Office Policies

We respect your time and ask that you respect ours. We require a minimum of 24 hours notice for any appointment cancellations or rescheduling requests.

• Failure to provide 24 hours notice will result in a standard $50 late cancellation fee.
• No-showing an appointment without notice will result in a $75 fee.
• Repeated cancellations or missed visits (3 or more) may result in discharge from our medical practice.

Note: These fees are not covered by health insurance and must be paid out-of-pocket prior to scheduling your next visit.

To ensure patient safety and proper medication monitoring, all routine prescription refill requests must follow these guidelines:

• Please allow 48 to 72 business hours for processing. Refills are not processed on weekends or holidays.
• We do not refill controlled substances or pain medications after hours or over weekends.
• Patients must remain compliant with scheduled follow-up visits. If you have not been evaluated in our clinic within the last 3 to 6 months, a routine refill may be denied until an appointment is completed.

All specialist copayments, past balances, and estimated co-insurance percentages are due explicitly at check-in prior to seeing your provider.

• We accept cash, personal checks, major credit cards, and HSA/FSA account cards.
• If your check is returned for insufficient funds, a $35 processing fee will be added to your account balance.
• Unpaid balances older than 90 days may be forwarded to an external collection agency if regular payment arrangements are not established.
