Billing & Payment FAQs
What am I expected to pay at the time of my visit?
All specialist copayments, outstanding deductibles, and co-insurance balances are due strictly at the time of service. For your convenience, we accept cash, all major credit cards, and Health Savings Account (HSA/FSA) cards.
What if you are out-of-network with my specific plan?
If Peak Performance does not participate directly in your network, you may still be seen utilizing your out-of-network benefits. Our billing department will gladly provide you with a formalized, itemized superbill to submit directly to your insurance company for reimbursement.
How do Workers' Compensation claims get processed?
For workplace injuries, we require your date of injury, case claim number, claims adjuster's name/contact info, and a formalized written authorization from your employer's workers' comp carrier before scheduling your treatment.
