1. New Patient Registration
Required for all patients establishing care with Peak Performance.
Complete New Patient Intake Packet
Includes demographics, medical history, pain mapping, and financial policies. Must be submitted prior to evaluation.
2. Procedure Consent Forms
Only complete these if explicitly instructed by your provider prior to an injection or treatment.
Interventional Injection Consent
Required for Epidurals, Facet Blocks, and Joint Injections.
Regenerative Medicine (PRP) Consent
Specific authorizations and disclosures required for Platelet-Rich Plasma clinical therapies.
3. Medical Records & Admin
Medical Records Release Authorization
Authorizes Peak Performance to request your past diagnostic images, X-rays, MRIs, and charts from your previous doctors.
